I confirm that I have arranged for the abovementioned child(ren) to be accompanied to the airport of departure and to be met at the destination airport by the person named in this form. These persons will remain at the airport until the flight departs and be waiting to receive the child at the arrival airport. Should the child not be met as stated on the face hereof, I authorise the carrier to take whatever action considered necessary to ensure the child's safe custody, including the return of the child to the departure airport.

I agree to indemnify and reimburse the carrier for such necessary and reasonable costs and expenses incurred by the carrier.

I declare that the child has all relevant documents( e.g. form of ID, passport, visa, health certificate etc) as required by the applicable laws.

I, the undersigned parent/guardian of the above-mentioned child(ren) agree to and request the unaccompanied carriage of the child and declare the information provided as accurate.
